The Silent Tragedy of the Unread ECG
A heart condition is not a death sentence—unless it goes unnoticed.
Imagine a patient in a remote rural clinic in an underserved region. An ECG is taken, but the tragic gap lies in its interpretation. Without a trained professional on site, the diagnosis is missed, treatment is delayed, and in the worst case, a life that could have been saved is lost. This is not a failure of medicine; it is a failure of access.
In Ethiopia, for example, there are only approximately 30 fully trained cardiologists for a population of more than 120 million people. The situation is similar in many developing regions around the world.
At MI4People, we believe in the power of AI. We have seen it create impact across borders, and this belief inspired us and our partners to ask a radical question: If experts cannot be everywhere, can AI bring their knowledge to the places that need it most?
Our answer was to live our mission: bringing AI for good into real-world healthcare. From this vision, SUSTAIN-AI ECG was born.
Our Solution: A Seed of Hope
SUSTAIN-AI ECG is a pilot project in Ethiopia that provides a web-based, AI-powered tool helping general practitioners detect 10 cardiac conditions from 12-lead ECG machines within seconds. It is built for real-world conditions:
Accessible: Runs in a standard web browser, requiring only a basic digital ECG device and an internet connection.
Supportive: Acts as a second pair of expert eyes for overworked general practitioners.
Empowering: The doctor is always in charge; the AI serves as a trusted assistant.
Free of charge: Designed to serve people, not profits.
From Seed to Sapling: A Bridge Built on Partnership
This project would not have taken root without the immense support of our community and partners. Technology alone does not save lives—people using technology do.
Our core technology partner, Technische Hochschule Mittelhessen (THM). The research group of Prof. Dr. Michael Guckert and Prof. Dr. Jennifer Hannig developed the AI system in their academic research.
Our local clinical lead in Ethiopia, the Armauer Hansen Research Institute (AHRI), provided the soil for growing the impact. Especially the project leader Minyahil Tadesse Boltena worked tirelessly to bring the tool into hospitals.
Crucial funding from the Bayerische Staatskanzlei (Bavarian State Chancellery), through the “Entwicklung in Partnerschaft (EiP) 2025” program, enabled the pilot to be deployed across the first hospitals.
The current pilot is running in nine Ethiopian hospitals under the supervision of local professional cardiologists. Thirty healthcare professionals have been trained to use the AI-ECG system, and it has already been applied in nearly 6,000 clinical cases.
At the same time, we are collecting anonymized ECG data to further improve the AI system. This dataset will also be made publicly available to foster research and support other organizations and startups. especially in developing regions, in building their own AI-based ECG tools. It will be the first dataset of its kind worldwide focused specifically on ECGs from African populations.
A Milestone Achieved: The Sapling Is Growing Strong
Today, we are proud to say: the system is live, operational, and already supporting real doctors in Ethiopia.

Not replacing doctors, but supporting them

Opening and sharing data, not locking it away for competitive advantage
The Next Chapter: Growing a Forest
We have planted the tree, and it is growing beautifully. Now, we need the sunlight and resources to grow it into a forest that supports every clinic that needs it.
Like many non-profit initiatives, we now face the challenge of scaling. The technological foundation is ready, but expanding to more hospitals requires additional resources.
